Setmelanotide is a synthetic cyclic eight amino acid peptide and a selective agonist of the melanocortin 4 receptor (MC4R). It is sold as a prescription medicine (Imcivree) in regulated markets for rare genetic obesity disorders involving the leptin melanocortin pathway and is studied as a research compound at MC4R.

In research models, Setmelanotide binds MC4R on neurons of the hypothalamus and signals through the melanocortin pathway that processes satiety inputs from leptin. The compound is selective for MC4R over MC1R, MC3R, and MC5R, which sets it apart from earlier melanocortin agonists in the same family.
